Harbor is seeking a licensed LPN to join our Toledo team! This position delivers care to clients with mental health and/or substance abuse.

Position is full-time, 40 hours per week, and 1st shift. Position works M-TH 8:30 - 5, and F 7:30 - 4:30.

Education/Experience/Other Requirements :

  • An Associate’s degree from an accredited school of Nursing with a current L.P.N. license in Ohio.
  • Two years’ experience working in psychiatric nursing preferred.
  • Must be proficient and accurate in computer use, including Microsoft Word.
  • CPR/BLS and NCI/CPI Certifications required within 90 days of employment.
  • Must have a valid driver’s license, acceptable driving record, and show proof of personal auto insurance policy to be deemed insurable with Harbor’s vehicle insurance carrier.

Essential Job Competencies/Primary Duties:

  1. Nurses may work at all Harbor sites but will have a site of primary responsibility. Responsibility for a site includes the following:
  2. reviews and updates treatment plans for clients receiving pharmacologic management services;
  3. orders and monitors Central Pharmacy medications;
  4. coordinating insurance forms and pre-certifications as necessary;
  5. Order/reorder/discontinue prescribed medication into medication module;
  6. completes quarterly checks of first aid boxes, bloodborne pathogen kits and maintenance of medical equipment; amount of time necessary to manage a site will be determined by supervisor and the nurse managing the site.
  7. Provides medication management of clients through:
  8. medication management appointments; in person/phone/TELE
  9. dispensing medication samples or Central Pharmacy medications;
  10. collaborating with other medical and clinical team members on an ongoing basis.
  11. Responsible for managing medication rooms, maintaining records of medication use and disposing of expired medications.
  12. When necessary, assists other clinical staff with crisis intervention and pre-hospital screening.
  13. Provides referral/linkage of client to appropriate resources.
  14. Completes the required number of peer review forms.
  15. Completes all necessary documentation in a timely manner.
  16. Types clinical documentation in the format appropriate and acceptable to Harbor.
  17. Provides support for providers by initiating lab requests and coordinating completion of patient medical documentation requests.
  18. In providing billable services, fulfills or exceeds unit expectations established by Harbor.
  19. Attends meetings as required and serves on committees as requested.
  20. Provides back up at other sites as needed and requested by supervisor.
  21. Maintains certification to do crisis intervention and pre-hospital screening.?
  22. Participates in professional meetings, workshops, seminars and other continuing education programs to keep current with professional practice, to contribute to his/her professional growth and to maintain licensure.
  23. May float to other locations to accommodate client or agency needs.
