Lebanon Center, a skilled nursing and rehabilitation facility in Lebanon, NH, is seeking a dedicated Nurse–LPN MDS Coordinator to support exceptional resident-centered care. In this role, you will coordinate and complete the Minimum Data Set (MDS) assessments for short-term rehab, long-term care, and memory care residents, ensuring accuracy and compliance with federal and state regulations. Working closely with nursing, therapy, social services, and medical staff, you will gather and validate clinical information, document care needs, and help develop individualized care plans that promote dignity, respect, and quality of life. The Nurse–LPN MDS Coordinator will monitor resident status, track outcomes, and update assessments as conditions change, supporting quality measures, reimbursement accuracy, and clinical decision-making. You will educate and collaborate with interdisciplinary team members on MDS processes, documentation standards, and resident assessment protocols, helping to drive continuous quality improvement. This position is ideal for an LPN with strong clinical assessment skills, attention to detail, and an interest in documentation, care planning, and regulatory compliance.
